Idebenone [2,3-dimethoxy-5-methyl-6-(10-hydroxydecyl)-1,4-benzoquinine] is a synthetic analog of CoQ10, the vital cell membrane antioxidant and essential constituent of th ATP-producing mitochondrial electron transport chain (ETC). Idebenone is a potent antioxidant, with the ability to operate under low oxygen tension situations. Because of its ability to to inhibit lipid peroxidation, idebenone protects cell membranes and mitochondria from oxidative damage. Its antioxidant properties protect against cerebral ischemia and nerve damage in the central nervous system. This compound has also been shown to stimulate nerve growth factor, a characteristic that could be important in the treatment of Alzheimer's and other neurodegenerative diseases.

Idebenone is six times more potent than CoQ10 in its antioxidant abilities. In fact, studies indicate the idebenone is 400 times more effective than Vitamin E or vinpocetine as a free radical quencher. Idebenone protects under hypoxic conditions, a situation where CoQ10 acts quite the opposite. This may be the most advantageous property of idebenone. When the body's oxygen level drops, such as during a heart attack, stroke, or surgery, low-oxygen conditions can cause a situation where CoQ10 may stop acting as an antioxidant and, instead, start producing free radicals. Idebenone however continues to function in low-oxygen environments, preventing damage to the body when it is most vulnerable. Superior low-oxygen performance is why idebenone is used to safeguard organs during transplants. In fact, idebenone is eight times more potent than aspirin as a blood-clot inhibitor.

Idebenone has been shown to increase Nerve Growth Factor and enhanced Cholinergic Activity, thereby showing it to have cognitive enhancement abilities. German studies showed that 90mg of idebenone administered daily to Alzheimer patients, thereafter showed statistically significant improvement.
